Job description:

The Tasmanian Health Service delivers the Transition Care Program (TCP), a short-term community-based service supporting older Tasmanian’s.

*Please note this position is also being advertised as an Allied Health Lead - Transition Care Program (519367) and only one position will be filled on merit.

The Team:

The Tasmanian Health Service delivers the Transition Care Program (TCP), a short-term community-based service supporting older Tasmanian’s after a hospital stay. Our multidisciplinary team provides person-centred care to help clients regain their independence and achieve their recovery goals. The Transition Care Program team partners with key stakeholders to deliver excellence in healthcare.

The Role:

The Nurse Manager of the Transition Care Program (TCP) leads an interdisciplinary team providing compassionate, accountable, respectful and excellent care. They provide strategic and operational leadership, overseeing human, financial and physical resources while ensuring compliance with Tasmanian Health Service and Commonwealth TCP guidelines as well as Aged Care Standards while guiding innovation and performance.

You will need:

  • Demonstrated capabilities in leading and nurturing an interdisciplinary team to deliver contemporary client focused services.
  • Proven leadership skills including the implementation of systems and process, engaging others, achieving outcomes, and driving innovation.
  • Demonstrated capability to manage resources including the use of information systems and data to support the efficient operation of the service, prepare reports, manage risk, meet quality standards, and inform change.

Details of appointment:

Permanent full time, day worker position, working 76 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $122,365 to $129,060 per annum. Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023

Eligibility:

Successful applicants will be required to meet the essential criteria.

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse with current AHPRA registration

Applicants should note the following criteria are desirable:

  • Drivers Licence
  • Postgraduate Qualification (or working towards) in relevant area.
